An estate plan will include two types of Power of Attorney: (1) Health Care Directive and (2) Durable Power of Attorney.

A Health Care Power of Attorney appoints designated individuals to act within a specific scope of authority delegated to them under the Heath Care Power of Attorney during your incapacity or after your death related to your health and medical care.

The Health Care Power of Attorney can be carefully tailored depending upon your wishes to address important decisions such as type and extent of medical care, hospice considerations, access to medical information, pain medication parameters, burial or cremation instructions, and other decisions related to your health and medical care.

Additionally, if you desire to avoid any extraordinary life-sustaining measures, especially those that would cause you pain or discomfort, a Living Will be drafted for inclusion within the Health Care Power of Attorney.

A Durable Power of Attorney appoints designated individuals to act within a specific scope of authority delegated to them under the Durable Power of Attorney during your incapacity or after your death related to your financial affairs.

The Durable Power of Attorney can be carefully tailored to your unique financial situation and addresses authorities such as signatory authority on financial accounts to ensure monthly bills are paid and properties are maintained, retirement accounts, stocks and bonds, money market accounts, investments, life insurance policies, and other financial matters.
